Suggestion for the Construction Site

The other day, while examining column-beam junction sections in ideCAD (Figure 1), it struck me. Since the design in the picture A in Figure 1 is not suitable, what kind of path do you follow on the construction site when you decide to do the one in B. For example, if we drop the spacers from both sides in such a beam, and if we drop the column reinforcement on at least one side, the stirrup width for the reinforcements becomes 25-3*2-1.4 cm, right? Or is the situation in Figure 1A not too bad as long as the cover covers are observed?. If I look back at the photos I took during my internship (Figure 2-3), I didn't like it at all. Also, in Figure 4, what do you do in your construction site in terms of square, stirrup design, spacers etc. for the 1 and 2 reinforcements.
