Does not carry cut-off safety

First of all, good day everyone. I am a final year civil engineering student. I'm working on a project, but I keep getting an error like "Does not carry shear safety, Maximum scoring exceeded, insufficient reinforcement". For shear safety and Maximum scoring, size of beams, reinforcement, wrapping stirrup etc. I've tried everything but it doesn't work. How can I adjust the reinforcements on beams and curtains? How can I recover these errors?
