Column positions are wrong in continuous foundation expansions

Column positions are wrong in continuous foundation expansions. For example, the column is located 367 cm from the beginning of the foundation in the plan. However, in the foundation beam expansion, the column is at the top. What is the reason of this. I see it very often, especially in foundations that are not parallel to planes. What could be the reason for this?
